IMPORTANT CHANGES FOR THE 2023-24 SCHOOL YEAR
Families need to complete a Certificate of Immunization Form (CIS), with a physician’s signature or provide medical records.
Other acceptable forms are a CIS filled out by you or another parent/guardian with medical records attached; a CIS printed by a health care provider or school from the Washington State Immunization System, or a CIS printed from the Department of Health online tool.

ENROLLING YOUR CHILD
Ocosta School District is using an online process to enroll new students.
The application process takes approximately 20-30 minutes. You can save as you go and do not have to complete it all in one sitting.
You need the following documents ready to upload into the online application to begin the enrollment process. You also have the option to take the required documents directly to the school. However, your child’s enrollment will not be approved until all documentation has been received.
Birth Certificate (Required)
Signed Certificate of Immunization Status (CIS) (Required)
Court Documents/Custody Arrangements/Parenting Plan/Adoption Decree/Restraining Order (Optional)
Once submitted online, the completed application is available for school approval.
